Abstract

The Portfolio Document (PD) is a structured resource developed to support care coordinators in compiling a comprehensive portfolio of evidence that demonstrates their competencies. Designed as a tool for both personal and professional development, the PD facilitates reflective practice, skill enhancement, and role-specific learning. By guiding care coordinators through the documentation of their achievements and capabilities, the PD serves as a critical instrument in fostering growth, accountability, and proficiency within the care coordination field.
